Along with what Will and oldangel said, here are some other factors.

The fact that Ralphy asked you to be in a big production, was probably
a gradual progression since he was asking for extras in his small
production a few days ago.
Also, NY is the center of things, and a while back you were bummed that
you were stuck in the middle of nowhere.

This follows into actually getting a shot at what you want to do, and
of course you naturally have a fear of failure which is not uncommon.
Dreams are ALWAYS wishes or fears in different forms.
Of course when you finally get your big break -if you're lucky enough
to-it's major, and you don't want to disappoint. Ergo , if you did
something on a big scale and failed your friends and family would know
and you'd feel even worse.

You're unconsciously thinking of your own mortality .
Could be all this Sir Jimmy/cancer stuff.
Also, the fact that you're young and when you think of dying
you think of 'old' and then you think of your grandma dying.
I'm sure it's weighing on your mind that he(Sir Jimmy) is so
young and yet had cancer.
Also, that brings to mind your other friends(who apparently died young).
I don't think it's any prediction that you're gonna die-just a fear.
It's natural.
