The porn name is your middle name (or, in some alternates, *first* pet's name) as your first name, and the name of the street that you grew up on as your last name. 'Full Frontal' confirmed this, because they used both alternates at different points, but it's been around forever.
I always thought my name, 'William Forest' (with whatever shortening of 'William' I felt like) was awesome (the alternate would actually be good too, 'Randy Forest', but my roommate totally has it beat. By the formula we knew, his porn name would be 'Long Elm'. I don't think that could ever be beat.
