It's possible, although extremely unlikely (in my opinion) that she dies in Episode III. I would guess she simply dies off-screen in between Episode III and A New Hope. There's too many other things going on, and loose ends that need to be tied up for her death to be seen. Not to mention that Leia remembers her mother, and that she died when Leia was "very young." I doubt Leia could remember her if she died in Episode III when the twins were only infants.

No. That's false. She's not anywhere in that film. I don't know how anyone could have come up with that.
